The number of individuals is rapidly decreasing because of illegal collection for cultivation. There is also direct damage by trampling and its highly restricted distribution is considered to pose a threat to the species (Kagoshima Prefecture 2003, Ministry of the Environment 2011). Habitat change caused by logging was mentioned as a threat in 1989 when the species was beginning to decline.

Arisaema kawashimae is listed on the 4th Red List of Threatened Plant Species of Japan, Vascular Plants (Ministry of the Environment, Japan 2012) as Critically Endangered. The species is designated as regional endangered wild fauna and flora by the ordinance of three towns on Tokunoshima Island in 2012, and collection is prohibited.
The species' habitat is covered by the Amamigunto Forest Ecosystem Reserve which was established in April 2013, as well as the special protection zone of the Amamigunto Quasi-national Park. Also, preparation for the designation of National Park is in progress.
