Major current threats to this species are collection for cultivation and trampling by tourists (Kagoshima Prefecture 2003, Okinawa Prefectural Government 2006). Because the species does not form bulblets, growth rate is low and it is estimated that many of the individuals in the wild plant market are illegally collected.

Arisaema heterocephalum ssp. heterocephalum is listed on the 4th Red List of Japan, Vascular Plants (Ministry of the Environment 2012) as Endangered, while A. h. subsp. majus (treated here as a synonym of the nominate subspecies). and A. h. subsp. okinawaense are listed as Critically Endangered. The subspecies on Amami-Oshima and Tokunoshima Islands are designated as regional endangered wild fauna and flora by the ordinance of municipalities, and collection is prohibited. The habitat of A. h. subsp. okinawaense is designated as Prefectural Natural Monument and Prefectural Nature Conservation Area and development activities are restricted.
