Allium aegilicum was described in 2000 and has been found at only two localities on the island of Antikythira (Greece), with very few individuals known up-to-date. There are no data on population size and trends but the species is considered not common and restricted to a very small area.

The species is threatened by changes in land use (infrastructure development: wind mills, photovoltaic (solar) panels). It might be affected by grazing and currently it has mainly been observed growing within shrubs. The known localities are very close (around 500 m from each other), so these threats can affect the whole population in just one event. Fire might also be a threat and if it occurs it can affect the shrubs protecting the species, which could then be grazed when they are less protected.
