The species is only known from five collections. Exploration of the Adelaide area of New Providence showed that there was a population in the woodlands around the fresh water areas in the area. An additional population was observed in 2014 and revisited numerous times in the Lower Bogue area of north Eleuthera. The population is small, fewer than 100 mature individuals.

Tourism driven urban development is the main threat to this species. There are five collections for Agave cacozela, one of them without geographical information, the other four are in two localities that are urban areas in the present. There are practically no natural spaces left in the island. The population of A. cacozela on Eleuthera is very close to a shore line and is less than 2 m asl; rising sea levels will likely cause periodic flooding from tidal and storm events likely altering the habitat with increased salinity.

The species is recorded in the Analysis of Conservation Status and International Trade "Succulent plants in trade from the Wild" (Fragoso et al. 1999) and its inclusion in CITES is proposed, given that it was found in trade and is threatened. Two specimens, one from New Providence and another from Eleuthera are now in the living plant collections in the Leon Levy Native Plant Preserve on Eleuthera.
