Zamia restrepoi has been recorded only in two nearby area, but the subpopulation in one of these localities has been destroyed. No detailed demographic study has been carried out for the species, but general field surveys suggest that the only extant known subpopulation is extremely small with less than 100 adults. It is possible that the species has more subpopulations, although many field surveys have looked for the species with no success, therefore further research on the population distribution and abundance is required.

The species has undergone a past population reduction. The type locality for this species was destroyed by flooding for the construction of the "Urra" Dam of the Sinu river (Cordoba province) around the year 2000. All individuals died, except for a few (close to 30 adults) plants that were rescued and relocated to a garden in the vicinity. In the year 2012, a new small subpopulation of the species was discovered in a locality near the original type locality. The newly discovered subpopulation is very small, with less than 100 adults remaining in two small forest patches (less than 10 hectares each). The only known extant subpopulation occurs in forest patches in a landscape highly threatened with deforestation, resulting from agricultural and urban expansion, mostly forest destruction for cattle ranching and crops. It is highly likely that the forest patches were this subpopulation persists will be destroyed in the next years.

Some conservation actions have been intended for the species, as it is listed on a threatened-species resolution in Colombia and in CITES Appendix II, therefore Colombian environmental authorities are required to prevent and mitigate negative impacts of human activities on this threatened species and to regulate its international commerce. In addition, Colombia launched in 2015 a conservation action plan for cycads, and in the last few years the “Colombian Cycad Society” has been working to promote population and habitat protection and restoration of small populations for Zamia species in the country, complemented with conservation actions related to research, monitoring, communication and education, among others.

Zamia restrepoi
 is currently known from only one very small subpopulation that persist in two small forest patches in a highly disturbed and unprotected landscape. When the subpopulation in the type locality was destroyed by the construction of a dam, some 30 adults were relocated to a protected site (with no native forest) built for taking care of rescued plants and animals, managed by an institution appointed by the dam administration. The rescued plants have reproduced in the garden up to this date, but most seedlings have not been nourished to obtain new adults. Previous to the destruction of the type population, seeds were distributed to several botanical gardens and private collections inside and outside Colombia, therefore the species is well represented in ex situ collections. A project has been running since the year 2013 to use plants propagated in the rescue garden and the remaining natural population and relocate them to a private reserve near the original type locality. Urgent actions are required to improve population and habitat viability in the long term. The conservation action plan for Colombian cycads has proposed specific conservation actions for the species, and there is a species conservation strategy, but the strategy still requires to secure funding for the long term.
