An estimated population of 2,300 individuals in the wild (29 individuals per km²) was included in the species description (Nelson Sutherland 2006). However, the subpopulation at the type locality has since been wiped out (A. Lindstrom and R. Steiner pers. comm.), a nearby subpopulation has also been wiped out, and a 3rd subpopulation contained very few individuals in 2012-2013. Two other subpopulations were known, but one has also been extirpated, leaving only one known viable subpopulation with 200 individuals.

This species is threatened primarily and existentially by rapid transformation of forest to cattle rangeland. There are no restrictions whatsoever in place to halt this transformation of habitat which has been demonstrated to result in complete elimination of subpopulations. Additionally, it is extracted by nurseries and sold as an ornamental in Honduras. It is also extracted by foreign collectors, possibly without permits.

This species is included in Appendix II of the CITES Appendices. Subpopulations of Zamia oreillyi do not occur in the buffer zone of the Pico Bonito National Park, but it has been erroneously stated in the past that they do (no subpopulation is known from any protected area). This species is critically threatened and needs a full set of actions to occur for it to survive. It is possible that unknown subpopulations exist within protected areas, and this should be confirmed as rapidly as possible.
