The only known population of this species is in southern George Town which is now in an advanced stage of residential development. This residential development has now fragmented the original population into three subpopulations. Currently we estimate there are between 400 and 500 individuals, 350 of which are mature individuals.

George Town's remaining fragments of natural forest are disappearing very rapidly, and Hohenbergia caymanensis is disappearing with them. The land on which this species grows is now highly valued for real estate. Most of the areas where the species used to be abundant are now covered with housing and landscapes of non-native plants.

Preservation of this species in its natural habitat should be the highest priority. This requires protection of the Ironwood Forest area in George Town. A conservation collection which has been established in the Queen Elizabeth II Botanic Park (QEIIBP) is derived from a few individuals and does not represent the genetic diversity still present in the wild. This collection should be expanded to serve as an effective conservation measure. Research is required to understand why this conservation collection is not reproducing and, in the meantime, seed banking is recommended urgently. This species is included in the Cayman Islands National Biodiversity Action Plan 2009 (Cottam et al. 2009).
