Though no comprehensive population assessment has been conducted for the species, it is noted to be locally abundant throughout its range, particularly in the Boston Mountains of Arkansas (Freeman 1975).

Specific threats to Ozark Trillium have not been adequately documented and additional research is needed to determine the level of impact of various potential threats. Threats to other Trillium species in the eastern United States include herbivory pressure associated with the overabundance of White-tailed Deer (Odocoileus virginianus) (Augustine and Frelich 1998, Dávalos et al. 2014, Goetsch et al. 2011, Horsley et al. 2003, Jenkins et al. 2007, Knight 2004, Rooney and Gross 2003, Thompson and Sharpe 2005, Webster et al. 2005), habitat fragmentation and disturbance associated with logging (Gonzales et al. 2006), indirect habitat degradation caused by invasive plants (Leege et al. 2010), and habitat degradation caused by feral pigs (Howe and Bratton 1976). Deer predation within the area is thought to present a low level threat. Logging, though potentially problematic, has been occurring within the region for centuries and the species remains common. Burning Bush (Euonymus alatus) and Bush Honeysuckle (Lonicera maackii) are present within the species' range and could cause population decline through competition, though at present, this is a minor threat.

Several protected areas occur within the range of the species including large sections of the Ouachita National Forest, Mark Twain National Forest, and several state parks (IUCN and UNEP-WCMC 2019). Additional research into the population size and trend, life history, and threats impacting the species is needed. The species is included in ex-situ collections at nine botanic garden sites (BGCI 2019).
