The species, though quite restricted in distribution, is locally abundant with thousands of stems described in some subpopulations (Gaddy 2008). The species occurs in four subpopulations. Slight population declines may be occurring as a result of erosion along levee banks.

Threats to the species are poorly understood. Collection for the ornamental trade has occurred, but the impact of such collection is thought to be minor. Several other Trillium species are under threat of herbivory from the overabundant white-tailed deer (Odocoileus virginianus) population (Augustine and Frelich 1998, Knight et al. 2009). White-tailed deer overabundance is quite common in the eastern United States and may impact the species within its range (Russell et al. 2001). Erosion of riverbanks is causing slow declines in population but is not thought to present a substantial threat. Feral hog (Sus domesticus) rooting has caused minor damage to some plants and may cause future declines. Chinese Privet (Ligustrum sinense) is present in the region and may negatively impact subpopulations of the species through direct competition and alteration of habitat.

The species occurs in three IUCN Category V protected areas within its range (IUCN and UNEP-WCMC 2019). Given its restricted range, population surveys and monitoring of population trajectories are warranted.
