The species is described as locally abundant within its range (Freeman 1975), but requires undisturbed forest conditions (Flora of North America Editorial Committee 2002, Lady Bird Johnson Wildflower Center 2019). Because such conditions are declining, the total population is likely to be in decline.

Remaining forests within the species’ range are highly fragmented by rights of way, pastureland, and urban development, a pattern which has increased in recent years (Rudis et al. 2008). Loss of mature, interior forest habitat is the largest threat to the species. Silvicultural practices, logging operations, and conversion of conversion of deciduous woodlands to pine plantations are also likely to reduce the habitat available to the species. Of the available land in the species range in Texas, only 1% is allocated to protected forest areas which are not accessible to logging.
Introduced Japanese Honeysuckle (Lonicera japonica), and Kudzu (Pueraria spp.) are known to negatively impact other Trillium species (Leege et al. 2010), and are present in large numbers within the species’ range (Rudis et al. 2008). Chinese Privet (Ligustrum sinense) and Japanese Climbing Fern (Lygodium japonicum) are thought to present a more substantial threat within this region. The impact of invasive competitors on Slender Trillium is not specifically known, but is likely to mirror related species in similar habitats. Feral hogs (Sus domesticus), which occur within the species’ range, have a negative impact on other Trillium species and likely lead to localized population declines in Slender Trillium (Bratton 1974). Excessive herbivory due to overabundance of White-tailed Deer (Odocoileus virginianus), a threat observed in several other Trillium species (Augustine and Frelich 1998, Jenkins et al. 2007, Knight et al. 2009), is likely to also impact Slender Trillium. White-tailed Deer overabundance is common throughout the eastern United States (Russell et al. 2001) and is promoted by several land use patterns evident in Slender Trillium’s range including forest fragmentation, silviculture operations, and the presence of adjacent urban areas and agricultural areas (Rudis et al. 2008). The specific impact of White-tailed Deer overabundance on Slender Trillium is currently poorly understood, but is likely to be causing ongoing population declines.

The species’ range includes a number of protected areas including Angelina National Forest, Big Thicket National Preserve, Sabine National Forest, Sam Houston National Forest, and Clear Creek Wildlife Management Area (IUCN and UNEP-WCMC 2019). The protection afforded by these areas is, however, somewhat questionable. The overwhelming majority of protected areas within the species’ range are logged periodically and provide only a relative degree of protection (Rudis et al. 2008). Conservation efforts should focus on the restoration of mature hardwood forest environments with appropriate fire regimes. The species is included in ex situ collections within three botanic gardens (BGCI 2019).
