There is no information on the population size of the species. However, a decline is suspected due to continuous loss of primary forests within its distribution range. The two known localities of the species are found in genetically isolated areas suggesting improbable genetic exchange, which is furthered by ongoing anthropogenic activities from these areas. The species is severely fragmented.

Pinanga heterophylla is primarily threatened by land conversion and habitat degradation caused by commodity-driven deforestation, mining, shifting agriculture and urbanization. A significant loss of tree cover in Negros Occidental (7,630 ha) and Albay (7,960 ha) were reported based on analysed data from Global Forest Watch (GFW 2020). Aside from these anthropogenic disturbances, natural calamities and other climate change events also threaten the forest of Albay which it is considered a high-risk province due to its locality (Grefalda et al. 2018).

Pinanga heterophylla is neither within a declared protected area (PA) nor included in the Philippine Red List. Thus, a species-specific in situ conservation programme should be developed. Site-based conservation approaches such as the declaration of critical habitat areas or local conservation sites where the species is found, are highly recommended. The species is known in at least three botanic gardens globally as ex situ collections (BGCI 2021).
