Plants are found in scattered colonies within its range, but never in great numbers. It was estimated for the 2003 and 2009 assessments that the population had declined by >50% over three generations and this original record of decline still falls within the period of three generations from the date of the current assessment. There has been no recent field work to determine whether the level of decline has stabilised or increased since the last assessment.

Macrozamia spiralis occurs in vegetation types that once dominated the areas west of Sydney Australia, including the Cumberland Plain where over 90% of the original vegetation has been cleared. Even though the vegetation in which M. spiralis occurs is included in several protected areas (Werakata National Park, Windsor Downs Nature Reserve, Agnes Banks Nature Reserve, Castlereagh Nature Reserve) these areas were proclaimed mostly between 1990 and 1999 (Agnes Bank was proclaimed in 1982) and the impacts of decline still affect the status of M. spiralis. Prior to the proclamation of these protected areas, the area was affected by land clearing, mineral extraction, timber harvesting, and firewood collection.

This species is listed in Appendix II of CITES. Plants occur in the Werakata National Park and in the Windsor Downs Nature Reserve, Castlereagh Nature Reserve and the Agnes Banks Nature Reserve. An education and awareness programme as well as the long-term monitoring of subpopulations will support the conservation of this species.
