This species is known from only two to three subpopulations, comprising in total between 1,000 and 2,500 mature individuals. Regeneration from seed seems to be limited.

Poaching of plants has occurred. The other main potential threats to Macrozamia occidua include inappropriate fire regimes which kill surface seeds and young seedlings and failure of the insect pollination mutualism.

This species is listed in Appendix II of the CITES Appendices. Most of the plants are protected in the Sundown National Park. An education and awareness programme as well as the long-term monitoring of subpopulations will support the conservation of this species.
