In Albania and Kosovo, the species is rather rare throughout its distribution range (Vangjeli et al. 1995) and fragmented in seven localities. In Albania, the plant went extinct from two other localities, Kulla e Ljumës and Drini i Bardhe, which were flooded due to the construction of a dam 1976. Currently, the species is facing threats on other three localities: Koritniku (Kosovo and Albania) and Surroj subpopulation in Albania. The population size in Albania and Kosovo was estimated to be between 800 and 900 mature individuals. Its population size in all countries is estimated to be c. 2,000 individuals. The minimum viable population (MVP) of the species is 45 in Podbregj locality, Koritniku mountain in the  Albanian side and c. 70 mature individuals were observed in Mali i Shenjte Mt. The larger subpopulation of 180 mature individuals was observed close to Surroj village in Albania. A decline of mature individuals was observed during a second monitoring, with 50 and 20 individuals in Surroj and Podbregj localities respectively. The reduction of the population size during the last 60 years was estimated to be more than 50%.

The Balkan Yam (D. balcanica) is threatened by fires, habitat fragmentation and destruction due to dam construction and deforestation. Three sub-populations in Albania and Kosovo are threatened also by quarrying activities and inundation following dam construction. The accidental fires that happened in 2007 are thought to have caused the decline of mature individuals in the Surroi subpopulation.

The species has been protected as a rare plant by the "Decree on the protection of certain flora and fauna species" in Montenegro (Official Gazette, MNE, No. 76/2006), and it is considered as Endangered species due to human activities in Kosovo, decision no. 239/86 of Institute for Nature Protection of Kosovo. It has been listed in the Red Book of Threatened and Rare Plants of Albania (Vangjeli et al. 1995) under ''R category, and as Endangered (A1b) in 2013 (MEA 2013).
