The known subpopulations are extremely small. In 2009, overall population size was estimated to be between 1,000 and 2,500 mature individuals. More recent surveys indicate much smaller population size: in China, (Gong et al. 2015) estimated there were 100 individuals, but in subsequent surveys (2019) no mature individuals were found in the four wild subpopulations and only 21 seedlings and juveniles were located (X. Feng pers com). There are several reports from Vietnam, which also indicate very small population size: a 2014 survey of locations in Vietnam indicated that there were approximately 40 individuals in two subpopulations (Y.-Q. Gong pers. comm. 2015); other independent surveys have recorded 50-100 individuals in the Tuyen Quang locality and ca. 50 individuals at the Yen Bai locality (Tien Hiep pers comm). Populations are difficult to survey so there may be a degree of undercounting and the population is therefore estimated to be between 150 and 250 plants.

Much of the forest habitat of Cycas multipinnata has been cleared or severely degraded. It now occurs mostly on steeper slopes or mountain tops where there is less threat from deforestation. This species is also highly sought after in the commercial plant trade and over-collection by commercial dealers has had a negative impact on wild subpopulations.

The species is listed in Appendix II of the CITES Appendices. Conservation measures need to address the significant loss of habitat and the impacts of plant harvesting. Cycas multipinnata was recorded in the Honghe Nature Cycad Reserve and in the Xilong Mountain Natural Reserve in Yunnan province, China but this does not seem to have prevented further decline in mature plants and only seedlings or juveniles have been recorded from China in recent years. An education and awareness programme as well as the long-term monitoring of subpopulations will support the conservation of this species.
