Cycas annaikalensis is known from two subpopulations consisting of the subpopulation at the type locality and a second subpopulation approximately 35 kilometres away. Surveys at the type locality have recorded approximately 300 plants of which only 30 were mature adults. The second subpopulation comprises 136 individuals of which 35 were classified as mature adults. There is insufficient information to provide any estimates of past or current declines in these subpopulations; however, it is subject to ongoing threats, including collection, and an ongoing decline is inferred.

The presently known habitat of this species is a highly disturbed reserve forest outside the protected area network suffering from cattle grazing, local hunting, unsustainable collection of forest products and frequent fire events (Balakrishnan 2017). In addition, Cycas annaikalensis suffers direct threat from collection of whole plants, leaves, cones and seeds for various purposes.
