Ceratozamia santillanii occurs in four subpopulations within and adjacent to the Ocote Biosphere Reserve. The largest subpopulation is estimated to have between 50 and 100 plants, and the remaining subpopulations all comprise fewer then 50 plants, mostly adults. The overall population size is therefore estimated to be between 200 and 250 individuals.

Ceratozamia santillanii is endangered due to the rapid transformation of its habitat into pastureland and corn and coffee plantations and as a result of illegal removal for ornamental purposes.

Ceratozamia santillanii is listed in CITES Appendix I. Several subpopulations occur in the Ocote Biosphere Reserve. There are plans to establish a small reserve to protect the habitat for this species.
