Cenchrus agrimonioides is rare, with a total known population of 157 wild individuals in seven subpopulations on the islands of Oʻahu and Maui. It is considered to be extirpated from the rest of its former range on Lanaʻi and in the Northwest Hawaiian Islands.

The major threats to this taxon include direct competition by invasive non-native plant species, and predation and habitat degradation by non-native animals, particularly feral pigs, cattle, deer, and goats, which degrade the habitat and harm the plants through predation, trampling, and rooting for food. Alien plants also threaten the species by altering its habitat and competing with it for moisture, nutrients, light, and space. Rats pose a threat to the species through predation of its plant parts and fruits. In addition, the taxon is threatened by fire and drought. Fortini et al. (2013) characterized this taxon as vulnerable to climate change (vulnerability index [0.409]).

The taxon is on the U.S. Endangered Species List, and also on the State of Hawaiʻi Endangered Species List. Two of the subpopulations occur in a protected area. The five Oʻahu subpopulations are managed and monitored by the Oʻahu Army Natural Resources Program and the two Maui subpopulations are managed and monitored by the Hawaiʻi State Plant Extinction Prevention Program. Ex situ cultivation, propagule storage is ongoing. Outplantings have occurred, and have produced a few viable offspring. Further research is needed on general ecology, life history, population size, distribution, and trends; population genetics, and threats.

Conservation actions such as fencing to exclude feral ungulates and control of alien invasive plants has improved the status of the remaining populations. Outplanting to augment subpopulations on the island of Oʻahu have greatly increased the number of plants. These are expected to increase the number of mature plants in the next several years, but the extent of occurrence and area of occupancy are unlikely to increase much more unless a subpopulation is established on another island. Seeds and living plants are kept as ex situ collections by the Oʻahu Army Natural Resources Program, the Fleming Arboretum, and Lyon Arboretum's Hawaiian Rare Plant Program.
