This species is rather widespread and appears mainly in very small subpopulations or even as single plants. However, there are strong populations in Crete, Rhodes, Cyprus and Malta where it can be rather abundant. Furthermore, in Spain, it is not rare in Extremadura, southern Andalucía and Murcia (Becerra Parra and Robles Dominguez 2009, Anthos 2018). The population size is not known. Declines have been noted particularly in coastal populations but the population trend is unknown (Delforge 1995, Bournérias and Prat 2005, Kretzschmar et al. 2007). In Tunisia it was suspected to be declining and almost extinct in the 1980s (Vallès and Vallès-Lombard 1988) but, after more intensive field researches carried out during the 2000s, it has been rediscovered in the country and even reported from new sites (Martin et al. 2015).

This species´ habitat quality and extent are in decline, mainly in the coastal populations, due to steadily increasing tourism, and many colonies have already been extinguished. Habitat destruction due to industrial development and land use changes are also threats to this plant (Delforge 1995, Bournérias and Prat 2005, Kretzschmar et al. 2007). Specimen collection is also considered a threat to the species although its magnitude is uncertain.

The species is present in protected areas (IUCN and UNEP-WCMC 2018). The orchid is included under Appendix II of the Convention on International Trade in Endangered Species of Wild Fauna and Flora and Annex B of the EU Wildlife Trade Regulations (CITES 2018). It is listed as Regionally Extinct in France (UICN France et al. 2010).
The following actions are recommended for some subpopulations, especially in North Africa (Delforge 1995, Bournérias and Prat 2005, Kretzschmar et al. 2007):
- Protection of the living individuals through legislation which bans the species from being picked or dug up.
- Ex situ conservation: Artificial propagation, re-introduction, seed collections.
- Monitoring and surveillance of the existing populations and sites.
- Estimate the population size and study their dynamics.
