There are areas where habitat loss due to farming has reduced the size of populations (Cole and Forrest 2011). For example, satellite imagery shows the expansion of agriculture along the Pakwach-Nebbi road, in Aloe butiabana habitat (Google Earth 2020). Around Lake Albert, the agriculture mainly consists of subsistence farming and small-scale cotton plantations (IISD 2015).

The species occurs within Semliki Game Reserve, and it was also sighted in the Murchison Falls National Park and the Kabwoya Wildlife Reserve (Cole and Forest 2011, UNEP-WCMC 2019). Murchison Falls National Park is Uganda's oldest and largest protected area. Commercially viable oil was discovered under the Albertine Rift in 2003 and since then oil extractions are ongoing in the national park putting biodiversity conservation at risk (MacKenzie et al. 2017). Recent plans to dam Murchison Falls have been abandoned by the Ugandan Government. 

Aloe butiabana
is known from three ex situ collections globally (BGCI 2022) and it is listed in CITES Appendix II. The species was not assessed for the Uganda National Red List (MTWA 2018).
