No quantitative population data are available for this species. However, it has been recorded as uncommon to rare by some past collectors and is known from only 8 collections, collected between 1963 and 1994. That said, Ellert (1998) noted that var. makurupiniensis is more frequent and widespread that var. ballii, being found in many of the lower gorges in the vicinity of the Makurupini River.

There is a potential threat to this species from increased fire frequency within its habitat, particularly where it is found among open rocky grassland. Mapaura (2002) notes that this habitat is burnt in dry years; many Aloe species are unable to tolerate regular burning. The subpopulations that grow on larger rock outcrops are, however, likely to be at least partially protected from fire. There is also a potential threat from horticultural collectors but this is likely to be minimal, especially at inaccessible sites.

No specific conservation measures or management plan are in place for this species, except that it is listed in CITES Appendix II. All of its known subpopulations occur within or on the margin of the Chimanimani National Park in Zimbabwe and the Chimanimani National Reserve in Mozambique.
