This species is known from only a single subpopulation, estimated at between 300 and 500 mature individuals (T. Rulkens and O.J. Baptista pers. obs.). The population is suspected to be stable. However, even though the population gives the impression of being in good condition, it is of concern that no young plants were noted in habitat and the entire population is composed of older, mature plants. No apparent cause for this lack of seedling recruitment was noted as, due to this species' unique situation of being surrounded by waterfalls and river rapids that offer the Aloe colony protection, grazing by animals would not seem to be a contributing factor (McCoy et al. 2017).

There are plans to construct a major hydroelectric plant on the Lúrio River that could possibly alter its flow (McCoy et al. 2017), but this is unlikely to have a negative impact on this uncommon Aloe as the proposed dam would be built a long way upstream from the type locality for this species and it is considered unlikely to be impacted by changes in river flow (T. Rulkens pers. obs.).
The natural isolation of the type locality appears to provide this species' population a level of protection, especially during the rainy season when the river level rises and it is not easy to travel along the bush tracks. Its favoured habitat of rock outcrops and thin soils also protects this species from agricultural activity and fire. The site may in future become an ecotourist destination due to its beauty, but as it is so isolated, it is unlikely to ever receive high tourist pressure (T. Rulkens pers. obs.).
The main concern regarding this species is the apparent lack of recruitment of new plants, as the population appears to comprise only mature individuals, but the causes of this are currently unknown (McCoy et al. 2017).

No specific conservation measures or management plan are in place for this species and it is not known to occur in any protected areas. Any negative pressures placed upon the aloes could rapidly degrade the colony, so vigilant monitoring in future would be advisable (McCoy et al. 2017). It is not known to occur in any ex situ plant collections (BGCI 2019). This Aloe is included on CITES Appendix II (CITES 2019).
