This plant is threatened by the destruction of its habitat by annual bush fire set for land-clearing and pastureland, and illegal collecting of living plant in their natural habitat.

Aloe acutissima is listed in Appendix II of CITES. It is recorded within four protected areas (Missouri Botanical Gardens, 2018): Andringitra, Bezà-Mahafaly, Isalo, Tsinjoriake (La Table/St Augustin). Seeds have been collected and stored in the Millennium Seed Bank, UK and duplicated in country at the Silo National des Graines Forestières (SNGF).
