The population is currently thought to be declining because the habitat of one locality has completely disappeared and, another one is currently facing selective logging. The species is currently known from two subpopulations in the northern part of São Tomé Island. Aerangis flexuosa might be present in the forest occurring within the Obò National Park. The number of individuals of the species is not known and cannot be estimated since the species is epiphyte.

The species is threatened by shifting agriculture at small-scale and selective logging for construction of houses and domestic uses. The species is known from three specimens, but one of these specimens was collected in a place that has been destroyed. It is thus known from two subpopulations representing two locations.

The species is rare with a restricted area of occupancy. It occurs in areas easily accessible by local residents. Its last collection was made in 2012 from a specimen cultivated in the Bom Sucesso shadehouse.
