Initial estimates were of very small and rapidly declining populations in Eswatini but J. Hurter (pers. comm.) reported larger subpopulations of at least 2,000 plants in Mozambique.

Encephalartos aplanatus is threatened due to the over-collecting of wild specimens, especially in the type locality in Eswatini. Concerns have been expressed about the loss of pollinators from remaining populations but this has not been verified. Beetle pollinators were collected as part of a genetic study published in 2008 (Downie et al. 2008) which indicated that E. aplanatus shares pollinator species with the nearby populations of E. villosus. Alien invasives at the type locality in Eswatini are also posing a threat to this population.

This species is listed in Appendix I of the CITES Appendices. There is a possibility that subpopulations may be found in the Mlawula Nature Reserve, Eswatini (based on the occurrence of a subpopulation near to the southern boundary of this reserve). Identification of suitable sites for the formal protection of this species is needed. An education and awareness programme as well as the long-term monitoring of subpopulations will support the conservation of this species.
